If you have recently moved into
the area, it is wise to register the whole family with the practice as soon as
possible. You will be registered with the doctor currently taking on new patients.
All new patients will be offered a consultation for a health check.
You will be given a new patient questionnaire. It is very important that you complete and return this to us as soon as possible. This may be the only information available to us until the arrival of your records from your old practice which may be several weeks.
ALL NEW PATIENTS WILL BE ASKED TO PROVIDE PROOF OF ADDRESS AND IDENTITY WHEN REGISTERING.
TEMPORARY PATIENTS/OVERSEAS VISITORS
UK residents staying in our practice area for only a short time may register with the practice on a temporary basis, and will be seen in the same way as fully registered patients. Visitors from overseas can also be seen as an emergency at the practice. Treatment for existing medical problems can be provided privately, which means there will be a charge for any consultation and treatment.

CHANGE MY DETAILS?
Please notify us immediately of
any change in your address. Apart from the obvious problems which may occur
if you need a visit from the doctor or a nurse, the Primary Care Trust could
remove your name from our list without your knowledg if you fail to keep us up
to date. Anyone who moves outside of our practice area will
be ask to re-register with another doctor in their new area. Under no circumstance
will anyone remain on our list if they have moved outside of our area.
Please ensure that we have your
current telephone and mobile phone numbers even if they are ex-directory. We
will keep these confidential and only divulge them to individuals or organizations
that need to know them in order to provide your health care. Many people frequently
change their service providers and phone numbers. We are often asked to pass
on urgent information to you by hospitals, and we occasionally get unexpected
abnormal test results requiring a fast response. Your safety could be at risk
if you do not keep us up to date.
