Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you need to complete the travel form below. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. Not all travel immunisation is available on the NHS and depending upon where you are travelling to you may be charged a fee for some injections. There is currently a national problem in obtaining vaccines for travel and we therefore may not be able to provide a complete service.

It is still important to complete the travel form as early as possible - at least 8 weeks before you travel. This will allow the practice nurse to review your application and provide advice on what vaccines are needed and what you may already be covered for.  Also when available these vaccines have to be ordered.

Please complete a Travel Form and return it to the surgery as soon as possible at least 8 weeks before you travel and a practice nurse will contact you to provide advice and book an appointment if required.
